Free VA Accreditation Reporting Forms

The filing requirements for Accredited Agents and Attorneys are found in 38 CFR ยง 14.629 (b) (iii) and (iv)

The VA does not mandate that accredited agents and accredited attorneys MUST use the forms provided below. The certification letters sent to the Office of General Counsel can be in any form, as long as the required information is provided. However, the Office of General Counsel has approved the attached sample forms, and they MAY be submitted.

The forms on the links below may assist accredited agents and accredited attorneys in fulfilling the VA's requirement that those who are accredited will certify in writing to the Office of General Counsel that they have completed the post- accreditation CLE requirements. A "Certificate of Completion" for any CLE course taken does not need to be submitted with the report.

Initial Certification
Must be filed within 12 months of initial accreditation

Annual Certification of Good Standing
Must be filed annually before the anniversary date of accreditation.

Three Year CLE Report
In addition to the Annual Certification of Good Standing, the 3-year CLE Report must be filed not later than 3 years from the date of initial accreditation and every 2 years thereafter.
